Collect and Showcase Custom Review Questions

Merchants can collect answers to customized questions when gathering product reviews. This feature allows buyers to filter reviews based on specific criteria related to the product, such as skin type, size preferences, etc. The collected answers can then be displayed on the product detail page to help future customers make better purchasing decisions.
Go to Settings > Review form in your AfterShip Reviews Admin
Step 1: Create a new custom form
Each store can create up to 50 custom forms.
Click Create new form.
Step 2: Set the form name and the “Show this form for” setting
The form name is for internal use only. Choose a name that helps you identify the form easily.
You can choose to apply the form to All products or to products with specific Shopify product tags.
If you select multiple product tags, any product with those tags will use this form for collecting and displaying reviews.

You can prioritize forms by dragging them in the list. The form at the top will have the highest priority, and only that form will appear if multiple forms are applicable.
If you enable two forms for the same Shopify product tag or all products, only the form with the higher priority will be displayed on the review collection page and review widgets. The other form will not be used. Therefore, you can only publish one active form for the Show this form for setting at a time.
Additionally, if you set a form with the Show this form for all products option in a high position, it will override any lower-positioned forms. To ensure fallback options, we recommend placing the Show this form for all products form in a lower position.

Step 3: Create custom questions for the form
The “Question”
This is the full question sentence shown on your review collection page to help reviewers understand the question.
Please make it clear and concise, avoiding overly long questions.
The “Question key”
This is the short identifier shown on your review widgets to help buyers read and filter the answers.
Make it straightforward and short.
This is also the unique key for the question, and it can be used to import and export reviews with customer responses.
Form limitations
Each form can contain a maximum of 20 questions.
Available question types
You can choose from several question types, with the option to set specific responses after selecting a question type:
Single-answer question

Multiple-answer question

Rating scale

Slider

Plain text

Required questions
If you mark a question as required, reviewers must answer it before submitting their review.
To ensure a high review collection rate, we suggest setting no more than 2 required questions per form.
Shown as review filter
If you set a question as shown under the review filter setting, it will be displayed on your review widget as part of the review content.
Only Single-choice and Multiple-choice questions can be shown as review filters.
Show average score in widget headers
If you set a question as shown under the show average score in widget headers setting, the average score will be displayed as part of the review summary on your review widget.
Only the Rating scale and Slider questions can be shown as review summaries.
Shown answer in review content
If you set a question as shown under the review content setting, it will be displayed as part of the review content on your review widget.
Single-answer, Multiple-answer, Rating scale , and Slider questions can be shown as review content.
Plain text questions cannot be shown as review content.
Step 4: Check the form preview and publish
Once you're satisfied with the form, click Publish to make the form active. It will then be available for use on review request emails, the review collection page, and review widgets.
Step 5 (Optional): Import historical review data
If you have already collected reviews using custom questions in another app, you can import that data via a CSV file. Follow the Import Instructions for seamless integration.
Ensure the Question key and Show this form for settings match your previous setup for successful importing.
If you set the review format in your review request email as Review online, your custom forms and questions will not appear in the review request email.
- Reviewers will fill out and submit the reviews, along with your custom form, on the review submission page.

If you set the review format in your review request email as Review first product in the email, the required questions from the form for the first product (usually the highest-priced product in the list) will be displayed in the review request email.
- After submitting the review with the required questions in the email, reviewers can still fill out additional optional questions on the review page (thank you page).

Write a review button form
You must set the Custom questions settings to Show custom questions in your Review widget editor to be.
Able to showcase these custom forms to your shoppers on the review form once they click the Write a review button to give their feedback.
Able to see answers to custom questions in the review details.
Able to see rating scale and slider question averages in widget headers.

To enable this feature, make sure to set the Custom Questions option to Show custom questions in your All Reviews widget editor. This will allow you to:

View responses to custom questions in the review details.
See average ratings and slider question results in the widget headers.

Once the review is verified and published by you, answers to custom questions will only be shown in the review details.
